ROCK ORCHESTRA PERFORMANCE AT HOUSE OF BLUES - PARENT TICKET ORDER FORM

    There are a limited number of seats for the lunch performance that we are making available for families who will be in Orlando to join us and watch the performance. The orders will be taken on a first come, first serve basis. The cost of each seat is $21.50.

                2. Provide a check made out to NHIPA for the cost of $21.50/person with House of Blues in the memo line. Please place the check in an envelope labeled House of Blues and deliver to either NHIPA Box.
